Post Calculating the velocity of Water

Hello everyone,
How can I calculate the speed of water sprouting out from a tiny hole on a bottle with the variables h, d , g (initial rate and if anymore variables are needed please write them too)? I would also like someone to prove the formula that is used for better understanding. Many thanks

A web page that talks about this is:
Bernoulli's Equation
in the section that begins:
"As an example of the use of Bernoulli's Equation, the classic problem of the velocity of efflux through a hole in the side of a tank."

It would be interesting to discuss the derivation on that page.
