You have a formula, and you should have data in your text book for values of G and M (the Earth's mass). As for the value of 'r' - it's measured from the center of the Earth, so is equal to the satellite's altitude plus the radius of the Earth. Now plug and chug - what do you get for 'v'? By the way, please check that formula - you have a factor of 2 in there which I don't understand.
